When the tide receded, the starfish were left behind and would die in the morning sun. While the tide was fresh, and the starfish were alive, the man took a few steps, picked one up and threw it into the water and continued throwing starfish back into the ocean. Behind him was a stranger who couldn't understand what this man was doing. He caught up with him and asked, "What are you doing? There are hundreds of starfish. How many can you help? What difference does it make?” The man did not reply, rather he took two more steps, picked up another one, threw it into the water, and said, "It makes a difference to this one.”With this story as his motivation and ultimate objective, it has always been Dr. Kotadia's goal, with each patient, to provide superior services.

Thomas Spina, MD

Dr. Thomas Spina is an adult psychiatrist who began his practice at Talkiatry after completing his psychiatry residency at the Eastern Long Island branch of Stony Brook University, where he treated a diverse patient population. Dr. Spina received his undergraduate degree from St. John's University in Queens where he studied Philosophy. Initially interested in a career in law, Dr. Spina decided to change gears and pursue a career in medicine after watching a relative struggle with her diagnosis of Diabetes Mellitus. Dr. Spina then completed medical school at Tel Aviv University in Israel, where he fell in love with psychiatry during his clerkship at Ichilov medical center. During that rotation, he came to appreciate how each patient has his or her own unique story, regardless of their diagnosis. Dr. Spina favors a holistic and integrative approach to psychiatry, combining medication management with supportive therapy, elements of cognitive behavioral therapy, while remaining open to other treatment modalities depending on the needs of his patients. When more focused therapy is indicated, Dr. Spina may recommend that his patient work with a provider specializing in a particular modality of therapy as part of their treatment. Dr. Spina treats adult patients with mood disorders including major depressive disorder and bipolar disorder, anxiety spectrum disorders, OCD, PTSD and specific phobias. Seth Mandel, MD

Dr. Mandel received his Bachelor of Arts and Doctor of Medicine degrees from the University of Pennsylvania. He completed his Residency in Psychiatry at Mount Sinai Medical Center in Manhattan. He has been board certified in psychiatry since 2001 and is a Life Fellow of the American Psychiatric Association. He has held numerous clinical and academic appointments including serving as Chairperson of Psychiatry at Huntington Hospital in Huntington, New York, and Assistant Professor of Psychiatry at Hofstra North Shore-LIJ School of Medicine.

Dr. Mandel has over 25 years of clinical experience treating all mental health conditions. He has a strong interest in mood and anxiety disorders, including work-related anxiety. He authored a section on the pharmacology of social anxiety in the book Work Makes Me Nervous. Dr. Mandel also has expertise working with clients diagnosed with borderline personality disorder. He has lectured on the pharmacologic management of this condition, including a presentation at the NIMH International Think Tank for the More Effective Treatment of Borderline Personality Disorder.

Beverly Bryant, DFAACAP

Dr. Bryant graduated from Mount Sinai School of Medicine to complete her adult training and the Yale Child Study Center/Riverview Hospital for Children Program to complete her fellowship in Child and Adolescent Psychiatry. A native Texan, she returned to Tyler, TX in 2020 to accept the position of Chair of the Department of Psychiatry and Behavioral Medicine at the University of Texas Health Science Center at Tyler. She also established an accredited child and adolescent psychiatry fellowship in Tyler. Prior to that, she served as the Director of the Child and Adolescent Psychiatry Division at the University of Mississippi Medical Center where she was also the Program Director of the Child and Adolescent Psychiatry Fellowship. She received her medical degree from the University of Texas Southwestern Medical School. Working with survivors of trauma has always been an important part of Dr. Bryant's practice. She currently serves as the co-chair of the American Academy of Child and Adolescent Psychiatry's Committee on Child Maltreatment and Violence and she is a representative for the National Health Collaborative on Violence and Abuse. Dr. Bryant also served as the program director of the Eating Disorders and Trauma Recovery program at Menninger Clinic in Topeka. She is board certified in adult, child and adolescent psychiatry and she has over 32 years of experience as a practicing psychiatrist. Dr. Bryant specializes in the treatment of anxiety disorders, obsessive-compulsive disorder, mood disorders, developmental disorders, Post-Traumatic Stress Disorder, Attention Deficit Hyperactivity Disorder, and family relational problems for both children and adults. Rae Insalaco, DO

Dr. Rae Insalaco is a board-certified psychiatrist who completed her residency in 2011 at the University of Rochester, where she served as the chief resident of ambulatory services. During this time, she also received the Richard Carl Albert Jaenike award for outstanding performance in clinical psychiatry. Dr. Insalaco uses the bio-psycho-social model as a blueprint to understanding her patients' challenges. Since residency, she has provided direct patient care in CPEP (Comprehensive Psychiatric Emergency Program) and outpatient, ambulatory clinics, treating a wide range of psychiatric illnesses. She has also served as the collaborating psychiatrist for psychiatric nurse practitioners, psychiatry residents, care managers, social workers, and therapists at a community mental health center affiliated with the University of Rochester. Dr. Insalaco completed medical school at the New York College of Osteopathic Medicine and continues to use the principles of osteopathic medicine in her care of patients. She believes that treating the whole person is important, and not simply treating your individual symptoms in isolation. Dr. Insalaco also examines how your habits of sleep, exercise, and nutrition can contribute to your overall wellness. Parker Loy, MD

Dr. Parker Loy is a board-certified Psychiatrist at Talkiatry. After finishing undergraduate studies at the University of Tennessee, Knoxville, he completed the University of Tennessee Health Science Center (UTHSC) Master of Science program before enrolling in medical school at the same institution. After graduating from medical school, he completed his residency at UTHSC, during which he trained in a variety of inpatient and outpatient settings and treated a broad spectrum of mental health conditions. He enjoys treating a wide variety of conditions, including mood and anxiety disorders. Kelvin McMorris, LISW

Kelvin is an experienced Licensed Independent Social Worker who has worked in a variety of clinical and non-clinical settings. Kelvin's years of experience include work as a case manager, mental health therapist, substance use and mental health group facilitator, a lead clinician for a school-based services program and coordinator for adventure-based programming. This experience has allowed Kelvin to serve in a variety of settings, including: schools, homes, hospitals, and outpatient clinics, as well as working with individuals from nearly every age range; anywhere from kindergarten-aged school children to adults in a nursing home and almost everywhere in between. ‍Kelvin places great importance on making treatment a unique experience for every individual, strongly believing that there is no single uniform answer to an individual's problems and solutions should be tailored to that person's specific life circumstances. Dr. Michael A. Murphy is board-certified in General Psychiatry as well as Child and Adolescent Psychiatry by the American Board of Psychiatry and Neurology. He has over 20 years of experience practicing medicine in outpatient, acute, and residential settings. He provides psychiatric assessments and medication management along with brief supportive psychotherapy. Stephanie Negrey, MD

She completed her undergraduate degree in Biology and History at Fordham University in the Bronx and then attended medical school at the Renaissance School of Medicine at Stony Brook University. She completed her residency at Wright State University in Dayton, Ohio as a member of the United States Air Force. Dr. Negrey served as an active duty psychiatrist; her duties included medical director of the mental health clinic at a special operations base as well as a program manager for the Air Force substance use program. Dr. Negrey has experience treating a wide range of ages and diagnoses, with particular emphasis on personality disorders, depression, anxiety, and post traumatic stress disorder. She integrates supportive and psychodynamic therapies into her medication management appointments with a goal of achieving improvement through a holistic approach.
