She completed her residency at Larkin Community Hospital, caring for a diverse range of patients. Dr. Safian diagnoses and treats a wide variety of conditions, including depression, anxiety, OCD, PTSD, trauma-related disorders, phobias, personality disorders, ADHD, bipolar disorder, postpartum depression, and concerns across all age groups. Michael Robbins is a psychiatric nurse practitioner with over 17 years of experience specializing in psychiatric medication management. He earned his MSN from the University of Tennessee in 2006. His career spans various settings, including acute inpatient care, private practice, community mental health, and substance abuse treatment. Michael treats conditions such as depression, anxiety, PTSD, OCD, bipolar disorder, and ADHD. He has worked both remotely and in-person, adapting to the evolving healthcare landscape. Mark Varela,MD

Dr. Mark Varela is a board-certified psychiatrist joining Talkiatry directly out of residency. He focuses primarily on medication management, often accompanied by brief cognitive, motivational, or supportive therapy. Dr. Varela received his medical degree and general psychiatric residency training both from Texas Tech University Health Sciences Center in West Texas. Namrata Shah, MD

Dr. Namrata Shah is a board-certified adult psychiatrist who completed her undergraduate training at the University of Wisconsin-Madison and earned her medical degree at the American University of Antigua. She completed her psychiatry residency at Creedmoor Psychiatric Center with training at New York-Presbyterian/Columbia University Irving Medical Center.

In addition to experience providing inpatient and outpatient care, Dr. Shah has worked with a wide variety of patient populations. The conditions she treats include depression, bipolar disorder, anxiety disorders, panic disorder, OCD, and ADHD. In addition, Dr. Shah is particularly interested in issues related to women’s mental health, including perinatal mental health issues.

Dr. Shah emphasizes the importance of a strong doctor-patient therapeutic alliance and a patient-centered approach. To provide the best care, she believes in compassion, open communication, and collaboration. As a primary treatment method, she combines psychopharmacological medication and supportive therapy.

Michele Schaefer, MD

Dr. Schaefer is a board-certified adult psychiatrist with over 16 years of experience. She earned her medical degree from the University of Nebraska Medical Center in Omaha, NE, and completed her adult psychiatry residency at Oregon Health and Science University in Portland, OR. She then specialized further by completing a forensic psychiatry fellowship at Yale University. She has worked as a staff psychiatrist at Atrium Health System for over 16 years.

Seth Flesher, MD

Staff Psychiatrist

Dr. Seth Flesher is double board-certified in adult psychiatry and addiction medicine.  Dr. Flesher’s practice focuses on medication management of psychiatric illness without the use of controlled substances.  Typically, he offers this in conjunction with supportive or emotionally focused therapy in 30-minute follow-up visits. However, occasionally Dr. Flesher may believe that additional therapy is also needed and ask that you bring a therapist into your care team to provide the best outcome.

Before joining Talkiatry Dr. Flesher was Medical Director of Advanced Psychiatric Services [complex/high profile cases] at Kings County Hospital and Unit Chief of the Covid19 positive unit.  He was also a clinical professor at Downstate Medical College where he was voted educator of the year numerous times by psychiatric residents and medical students.  Dr. Flesher is a graduate of the prestigious combined 6-year BS/MD program at The Sophie Davis School of Biomedical Education.  He did research at the neuropsychiatric branch of the National Institute of Mental Health, completed his psychiatric residency at St. Lukes/Roosevelt Hospital, and he received his master’s degree in public health from Columbia University.

Dr. Flesher believes that psychiatry should start with a thorough assessment to arrive at an accurate diagnosis.  Then a discussion with the patient of the different evidence-based options available.  Followed by shared decision-making to arrive at an effective treatment plan to achieve the goals that the patient identifies as being important.  This should be done in an honest, open, non-judgmental, but realistic environment.  Understanding that the ultimate goal is to get the patient to a position where they will no longer need medication and/or therapy.
