Jonathan Hertz, MD

Dr. Jonathan Hertz is board-certified in adult psychiatry. He completed fellowships in child and adolescent and public psychiatry and has been practicing psychiatry for over 18 years. Dr. Hertz's practice focuses on medication management which is offered with supportive and/or dynamic psychotherapy within 30-minute follow-up sessions. If patients have individual psychotherapists, Dr. Hertz will work collaboratively with these providers within a treatment-team approach to patient care delivery. Dr. Hertz received his medical degree from Tufts University School of Medicine in Boston. He remained in Boston to complete a residency in adult psychiatry at Cambridge Health Alliance and a fellowship in child and adolescent psychiatry at Children's Hospital Boston. Dr. Hertz subsequently returned to his hometown of NYC and completed a fellowship in public psychiatry. Dr. Hertz has clinical and administrative experience in the field of psychiatry. These positions include ACT Team community psychiatrist, director of adult outpatient clinic, and medical directorship in behavioral health within community hospitals. With training in child/adolescent, adult, and public psychiatry, Dr. Hertz has a developmental approach to mental health care. Rachel Klein, MD

Dr. Rachel Klein received her medical degree from the UCLA School of Medicine and completed her residency training at Harbor-UCLA. She went on to complete a Consultation Liaison fellowship at Yale New Haven Hospital. Dr. Klein has served as a Consultation Liaison psychiatrist providing psychiatric care to patients across hospital settings and in emergency rooms. She has also worked in community health, delivering outpatient psychiatric care to a diverse range of patients.

Dr. Klein provides care in a supportive, non-judgmental, and empathetic manner. She works collaboratively with patients to identify goals and treatments that are effective, safe, and minimize the risk of dependence. Her approach emphasizes education about mental health and the use of non-pharmacological strategies to enhance wellbeing. She values treating the whole person and considers physical health as an integral part of understanding emotional distress.

Chhavi Chaudhary, MD

Dr. Chhavi Chaudhary is a dual board-certified psychiatrist with subspecialty certification in Child and Adolescent Psychiatry. She completed her fellowship at Washington University in St. Louis, where she also served as Chief Fellow, and her general psychiatry residency at the University of Texas Health Science Center at Tyler. Over the past year, she has provided telehealth psychiatric consultations across emergency rooms and inpatient settings, gaining extensive experience across state hospitals, outpatient clinics, college mental health, preschool, mood disorder, and community health clinics.

Patients working with Dr. Chaudhary can expect a thoughtful and collaborative approach. Nan Ambrosy, PMHNP

Nan Ambrosy is a PMHNP with over 23 years of experience in outpatient and inpatient settings, caring for patients across the lifespan with a wide range of diagnoses. She earned her Doctor of Nursing Practice from Graceland University and holds master’s and post-graduate certificates from Wichita State University. She is ANCC board-certified as both an Adult Psychiatric Clinical Nurse and Lifespan Psychiatric Nurse Practitioner.

Nan uses a patient-centered approach with shared decision-making, fostering strong therapeutic relationships. She focuses on the whole person, emphasizing a balanced lifestyle that includes physical health, sleep, nutrition, exercise, social connections, and spirituality.

Seth Flesher, MD

Dr. Seth Flesher is double board-certified in adult psychiatry and addiction medicine.  Dr. Flesher’s practice focuses on medication management of psychiatric illness without the use of controlled substances.  Typically, he offers this in conjunction with supportive or emotionally focused therapy in 30-minute follow-up visits. However, occasionally Dr. Flesher may believe that additional therapy is also needed and ask that you bring a therapist into your care team to provide the best outcome.

Before joining Talkiatry Dr. Flesher was Medical Director of Advanced Psychiatric Services [complex/high profile cases] at Kings County Hospital and Unit Chief of the Covid19 positive unit.  He was also a clinical professor at Downstate Medical College where he was voted educator of the year numerous times by psychiatric residents and medical students.  Dr. Flesher is a graduate of the prestigious combined 6-year BS/MD program at The Sophie Davis School of Biomedical Education.  He did research at the neuropsychiatric branch of the National Institute of Mental Health, completed his psychiatric residency at St. Lukes/Roosevelt Hospital, and he received his master’s degree in public health from Columbia University.

Dr. Flesher believes that psychiatry should start with a thorough assessment to arrive at an accurate diagnosis.  Then a discussion with the patient of the different evidence-based options available.  Followed by shared decision-making to arrive at an effective treatment plan to achieve the goals that the patient identifies as being important.  This should be done in an honest, open, non-judgmental, but realistic environment.  Understanding that the ultimate goal is to get the patient to a position where they will no longer need medication and/or therapy.
