Dr. Raafay Deen is a board eligible adult psychiatrist. At the heart of Dr. Deen's clinical approach is a strong emphasis on establishing trust and building a therapeutic relationship to help patients achieve their goals. Dr. Deen's practice focuses on medication management. Typically, he offers this in conjunction with supportive or emotionally focused therapy in 30-minute follow-up visits. Occasionally, Dr. Deen may believe that additional therapy is also needed and ask that you bring a therapist into your care team to provide the best outcome .Dr. Deen received his medical degree from Lake Erie College of Osteopathic Medicine and went on to complete his residency in psychiatry at Westchester Medical Center. He has practiced in various settings over his career, including inpatient psychiatric units, outpatient clinics, emergency rooms, and inpatient substance abuse rehab/detox programs where he treated patients with a variety of psychiatric illnesses. Caitlin Tillberg, MD

Dr. Caitlin Tillberg (she/her) is a board-certified psychiatrist. She has a BA in musicology and cognitive science from Oberlin College, an MD from Quinnipiac University in Connecticut, and completed general adult psychiatry residency at Duke University. Dr. Tillberg practices patient-centered and trauma-informed care, valuing patient autonomy and collaborative decision-making, acknowledging the burden many people carry from intersecting oppressions. She believes therapy is important � often equally important as medication, or more � and will work together with you to make a realistic treatment plan that will fit into your life. Dr. Tillberg also has a niche working with people with complicated relationships with their bodies, whether due to disordered eating, gender dysphoria, chronic illness/disability, or trauma. She welcomes LGBTQ+ patients, including trans and non-binary adults. She also enjoys working with patients with OCD, as well as autistic/neurodivergent adults (including those seeking adult autism evaluation). Please note Talkiatry is most appropriate for autistic patients with Level 1 (or case-by-case Level 2) support needs and who are verbal. Regarding eating disorders: Please note that for patient safety, per Talkiatry policy, Dr. Tillberg is unable to work with patients with current active anorexia nervosa or bulimia nervosa (or significant relapse in the last 6 months), or with patients requiring current regular medical or lab monitoring for complications of binge-eating disorder or ARFID. Bariatric surgery patients are welcome if medically stable. Patients with current disordered eating that does not rise to the level of a formal diagnosis of anorexia or bulimia, or with a past history of anorexia or bulimia in sustained remission, will be evaluated on a case-by-case basis at the first appointment. Jackie Long, MD

Dr. Jackie Long is an adult psychiatrist who recently completed her residency at the Institute of Living. During her training, she received extensive training caring for a diverse patient population in a variety of settings including emergency, inpatient, partial hospitalization, and outpatient settings.

Dr. Long blends clinical expertise with a holistic, thoughtful approach individually tailored to each patient’s needs. At the core of Dr. Long’s approach is a belief that healing is most effective when it is collaborative, personalized, and grounded in a strong therapeutic relationship.

Dr. Long provides medication management combined with supportive therapy. She treats a variety of patients with a focus in mood and anxiety disorders and has a clinical interest in cross cultural psychiatry.

Megan O'Brien, PMHNP

Staff Nurse Practitioner

Megan O’Brien is a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP) with experience in inpatient, outpatient, and emergency mental health settings. She earned her NP degree from UCSF, the top-ranked program in the country at the time. In addition to clinical work, she enjoys teaching and is passionate about training the next generation of compassionate psychiatric nurse practitioners.

Megan believes in meeting patients where they are, offering empathy, respect, and acceptance. She values open communication and sees treatment as a collaborative process. Her approach empowers patients to take the lead in their care while she supports them as a partner and guide along the way.

Megan begins by listening carefully to each patient’s story and asking relevant questions to understand their needs. Together, she and the patient build a tailored treatment plan that aligns with their goals. Her focus is on reducing mental health symptoms and helping people experience greater freedom and flexibility in their lives.

Amrish Pipalia, MD

Staff Psychiatrist

Dr. Pipalia is board-certified in both adult psychiatry and internal medicine. Since graduation from residency, he gained specialized experience in treating veterans with terminal illness, specifically Amyotrophic Lateral Sclerosis (aka Lou Gehrig's Disease), at the VA Medical Center in Portland, OR. He combines both psychotherapy and medication treatment to provide holistic care that addresses not only your mental health but also the parts of your life that surround and are deeply intertwined with your mental health. When more intensive psychotherapy is warranted, he collaborates with an experienced psychotherapist to serve you better. With his experience of internal medicine, he is also comfortable with navigating complex medical issues that may interact with your mental health. Dr. Pipalia is a graduate of St. George's University School of Medicine. He completed a combined Psychiatry & Internal Medicine residency training program at East Carolina University. He went on to complete a fellowship in Clinical Informatics at Oregon Health & Science University, learning how to use modern digital technologies and patient data to improve patient care. His academic activities include a presentation he has given multiple times on Illness Identity, teaching other clinicians how having an illness can change how we perceive our place in the world and how to help patients manage their relationship with illness.

Elyse Sipka, PMHNP

Elyse Sipka is a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP) with over a decade of experience as a mental health RN in outpatient, inpatient, and residential settings, including eating disorder treatment centers, inpatient psychiatric units, and crisis units. She earned her Master of Science in Nursing from Indiana Wesleyan University in 2021 and has been practicing as a PMHNP in an outpatient setting for over four years, caring for adults with a wide range of mental health needs including depression, anxiety, OCD, bipolar disorder, ADHD, PTSD, substance use, eating disorders, and severe mental illness.

Elyse creates a safe and supportive environment where patients feel heard and valued. She is warm, empathetic, and approachable, providing comprehensive and holistic care that combines medication management, therapy, and lifestyle interventions to help clients achieve their treatment goals.

Elyse begins with an in-depth initial assessment exploring physical, mental, emotional, and spiritual health. She collaborates with clients to develop individualized treatment plans, integrating medication management with psychotherapy using principles of supportive therapy, Acceptance and Commitment Therapy (ACT), and Cognitive Behavioral Therapy (CBT).

Catherine Cashman, DO

Background and credentials    

Dr. Catherine Cashman is triple board-certified by the American Board of  Psychiatry and Neurology in Adult Psychiatry, Child & Adolescent  Psychiatry, and Forensic Psychiatry. She holds two Bachelor’s degrees: one in  Finance from New York University, and another in Biology from the University  of New Orleans. She earned her medical degree from Nova Southeastern  University and completed an Adult Psychiatry residency at LSU Health Sciences  Center in New Orleans. Dr. Cashman subsequently completed two fellowships:  the first in Child & Adolescent Psychiatry at LSU New Orleans, along with  a certification in Infant Mental Health (working with children ages 0–6 years  and their caregivers); the second fellowship was in Forensic Psychiatry at  the University of Rochester and the Rochester Regional Forensic Unit.
