How do I request my medical records?
If you need a copy of your medical records from Talkiatry, you’ll need to download and fill out this HIPAA form. Once you’re done, send the completed form to medicalrecords@talkiatry.com. You’ll need to fill out a copy of this form to each individual party you’d like us to share your records with.
