How do I request my medical records?

If you need a copy of your medical records from Talkiatry, you’ll need to download and complete the first form in our documents center to give us permission to release your medical records, pursuant to HIPAA.

Once you’re done, send the completed form to medicalrecords@talkiatry.com. You’ll need to fill out a copy of this form to each individual party you’d like us to share your records with.
