Dr. James Ritchie, DO

I am trained in both adult and child & adolescent psychiatry, with a focus on the diagnosis and treatment of depressive disorders, anxiety disorders, trauma- and stressor-related disorders, and ADHD. I emphasize a holistic approach, combining medication management and psychotherapy, and I utilize principles from Cognitive Behavioral Therapy (CBT), Acceptance and Commitment Therapy (ACT), and supportive psychotherapy. This method is supported by research indicating improved patient outcomes.

I received my bachelor's degree in biology with a minor in French from The University of Central Florida. I completed medical school at Campbell University School of Osteopathic Medicine, followed by a General Psychiatry residency and Child and Adolescent Psychiatry Fellowship at Texas Tech University Health Sciences Center. My professional experience includes working in inpatient psychiatric hospitals, consult-liaison psychiatry, outpatient clinics, intensive outpatient programs (IOP), partial hospitalization programs (PHP), and emergency rooms. This broad range of experience allows me to provide comprehensive, individualized care to patients at all stages of life.

Patients can anticipate a thorough initial evaluation during their first visit with me. I conduct a comprehensive assessment to understand the patient's symptoms, history, and personal circumstances. This session sets the foundation for collaboratively developing an individualized treatment plan focused on the patient's specific needs and goals.

I like to work with patients on a customized treatment plan with active patient participation. Treatment goals are defined and updated as needed in follow-up visits. I encourage my patients to ask questions and learn more about their condition; communication and education are essential for treatment success. My areas of interest include mood and anxiety disorders, cognitive and neuropsychiatric disorders, PTSD, bipolar disorder, OCD, psychopharmacology, treatment-resistant conditions (depression), and psychoeducation to improve treatment compliance.

I joined Talkiatry as a general adult psychiatrist. I earned my medical degree from the Faculty of Medicine, University of Cairo, Egypt, and completed my residency in general psychiatry at Case Western Reserve University and MetroHealth Medical Center in Cleveland, Ohio. I have practiced psychiatry for 30 years in a variety of settings: hospitals, inpatient and consultation-liaison to other medical services, private practice, and outpatient care treating adults, geriatric patients, and adolescents. I have also provided geropsychiatric consultations to long-term care facilities and assisted living.

For new patients, I will conduct an initial evaluation; providing treatment records can be helpful. I will share treatment options with the patient and create a custom treatment plan. Follow-up appointments are scheduled every two weeks at the beginning of treatment.

My approach is rooted in compassion, collaboration, and respect for your unique story. I view mental health treatment as a partnership, one where psychotherapy and medication can work together to support your growth and healing. My goal is to help you uncover the patterns and behaviors that may be holding you back, and empower you to make meaningful, sustainable change. I encourage patients to take ownership of their mental health, and I am here to provide the tools, guidance, and support you need along the way.

We'll begin by taking the time to understand your story, your goals, and what brings you to treatment. The first few sessions focus on building trust and identifying the areas where you'd like to see change. From there, we'll collaboratively develop a treatment plan that may include psychotherapy, medication, or both, always tailored to your needs. I draw from multiple therapeutic approaches, including Cognitive Behavioral Therapy (CBT), Cognitive Processing Therapy (CPT), and Psychodynamic Psychotherapy, to provide care that fits you, not the other way around. Throughout our work, I'll support you in developing insight, resilience, and confidence in managing your mental health.

My background includes medical training at the University of Utah, where I served as Student Body President, and psychiatric residency at the University of Texas, where I served as Inpatient Chief Resident and received awards for both clinical excellence and medical education. I hold an MD and an MBA.

Outside of clinical care, I am deeply interested in narrative medicine, the way our personal stories shape our mental health, and I found my passion for mental health care through the power of listening to people's stories. I bring that same commitment and curiosity to my work with patients.

I strive to create a supportive, collaborative space where patients can explore what's bringing them in and work together to determine the best path forward. Whether someone is new to mental health care or returning after some time away, I recognize that it's normal to feel a mix of emotions at the start. I take a thoughtful, personalized approach to treatment that may include medication, therapy recommendations, or simply creating space to talk. I value open communication and believe deeply in each person's capacity for meaningful change. Follow-up visits are 20 to 30 minutes, focusing on monitoring progress and adjusting care as needed. I also value collaborative care and often coordinate with therapists or refer patients to therapy when additional support can enhance outcomes. While telehealth offers flexibility and convenience, I am committed to building a meaningful connection with each patient. I do not provide emergency services between sessions but ensure patients can access the appropriate resources when needed.

I am an Adult Psychiatrist who completed my psychiatry training at Texas Tech University Health Sciences Center in Lubbock, Texas. I served my program as a chief resident, which helped to build my leadership and interpersonal skills, and I dedicated myself to leading my resident team and fostering a supportive learning environment. I received the award of excellence in psychiatry knowledge during my residency training. I also hold a Master's in Public Health from Texas Tech University Health Sciences Center and am a Phi Kappa Phi Honor Society member. As someone early in my career, I bring fresh training and a strong commitment to transparency, lifelong learning, and evidence-informed care.

I believe that each individual's background plays a significant role in their mental health, and that understanding a patient's personal story is critical to their healing journey. I believe in a respectful, collaborative relationship built on trust and open communication. While I integrate evidence-based medication management when clinically appropriate, I also emphasize the therapeutic benefit of addressing lifestyle, relationships, and internal narratives. I begin care with a thorough evaluation, exploring developmental history, impactful life events, and emotional patterns. From there, I develop a comprehensive treatment plan that may include medications, wellness strategies, and therapist collaboration. I consider each patient's strengths and challenges to guide care that promotes long-term recovery and personal growth.

I completed my psychiatry residency at TriStar Centennial Medical Center in Nashville, Tennessee, where I served as Chief Resident. I earned my medical degree from Western University of Health Sciences College of Osteopathic Medicine of the Pacific Northwest in Oregon, where I studied osteopathic medicine, which shaped my holistic view of health. I place a strong emphasis on establishing meaningful relationships with my patients and supporting them in achieving their individual life goals through effective treatment. I actively incorporate patient preferences into all treatment plans and integrate psychotherapy with medication management. I recognize the importance of educating patients about their care and dedicate time during each appointment to address any questions or concerns they may have.

In my clinical practice, I begin by facilitating an open dialogue with patients regarding their primary concerns for the appointment. During initial consultations, I conduct a comprehensive assessment, which includes gathering relevant medical, familial, and social histories that may influence the course of treatment. In subsequent appointments, I engage patients by inquiring about developments that have occurred since their last visit, as well as any issues related to medications or significant life events.

I specialize in treating depression and anxiety. I acknowledge that while medications can be necessary, they are not magic, and a holistic approach is essential for achieving mental health. With this understanding, I spend time getting to know my patients, focusing on helping them achieve mental wellness rather than simply prescribing medications. When I became a doctor, I made a pledge to "do no harm." I take this commitment seriously and only prescribe medications, especially those with addictive properties, when it is safe and clearly indicated.

The first one or two appointments focus on a thorough assessment to establish a provisional diagnosis, which is essential for effective treatment planning. Once this initial diagnosis is made, I collaborate with my patients to develop tailored treatment recommendations. During follow-up appointments, I actively listen to my patients and propose adjustments to the treatment plan as needed.

I tend to have a relaxed but professional manner with patients. I prioritize understanding patient concerns and providing education, and I aim to use my medical expertise to advise you as we work together to create a strategy that will empower you to take charge of your mental health. I have a particular interest in perinatal/reproductive psychiatry and am pursuing additional training and certification in this subspecialty.

I completed medical school at The University of Oklahoma College of Medicine and completed psychiatry residency at The University of Oklahoma School of Community Medicine. Throughout my training, I have been involved in advocating for patients both at the state level as well as through my involvement with the American Medical Association.

New patients should expect a thorough review of their history and symptoms as they are establishing care with me. One of the most important steps to helping you get better is to make sure the diagnosis is correct. From there, I will work with patients to find appropriate medication strategies, as well as other evidence-backed interventions like therapy and lifestyle changes that fit your unique situation.

My approach focuses on behavioral change, with medication management and supportive therapies serving as important (and often essential) adjuvants to optimizing mental health. I have additional training in Women's Mental Health, Reproductive and Perinatal Psychiatry, PTSD, and trauma-informed ADHD evaluation and treatment.

I completed additional training in Psychiatry at the University of Washington after working primarily in women's health, primary care, and military medicine as a Battalion Surgeon in the United States Army. Following my military service, I trained in Psychiatry and served the mental health needs of our Veteran population.

During your first meetings with me, you should expect a thorough evaluation of your clinical and social histories, as well as some introductory psychoeducation. Following that screening, anticipate a discussion about my approach to your condition and an initial treatment plan.

I believe that the key to success in mental health care is being able to establish a trusting and open relationship between psychiatrist and patient. I feel comfortable working with a wide range of age groups and enjoy treating challenges with anxiety, mood, sleep, and concentration. While initial sessions include the need to gather background information about the patient overall as well as the symptoms of concern, subsequent sessions are focused on learning the impact of the treatment.

I am a board-certified psychiatrist who attended the University of Iowa Medical School and completed my psychiatry residency and child and adolescent psychiatry fellowship at Walter Reed National Military Medical Center. I have extensive experience treating active-duty service members, family members of active duty, and the veteran population.

Dr. Garrick Gaffney, DO

I specialize in treating complex psychiatric conditions, including depression, anxiety, PTSD, OCD, bipolar disorder, and insomnia. As a proponent of holistic care, I integrate psychotherapy into medication management and emphasize lifestyle modifications and evidence-based integrative medicine. New patients receive comprehensive evaluations to rule out medical causes of symptoms, followed by tailored treatment plans combining therapy, medication, and lifestyle adjustments. Follow-up visits focus on symptom management through supportive, brief dynamic, or cognitive behavioral therapy, with frequent appointments until stabilization is achieved. I am committed to collaborative, patient-centered care that addresses both mental and physical well-being.

I am board certified in psychiatry by the American Board of Psychiatry and Neurology (ABPN). I earned my medical degree from Kansas City University and completed my psychiatry residency at John Peter Smith Hospital in Fort Worth, Texas. During medical school, I pursued advanced training as an Osteopathic Manipulative Medicine fellow. As a resident, I served as Chief Academic Resident and earned accolades such as Intern of the Year and Outstanding Educator of the Year twice consecutively. I have presented case studies and quality improvement projects at national conferences, including the American Psychiatric Association (APA), Academy of Consultation-Liaison Psychiatry, and Association for Academic Psychiatry.

A combat veteran, I served in the U.S. Marine Corps, including a tour in Iraq, and was honored with the Navy and Marine Corps Achievement Medal and a Certificate of Commendation for exemplary leadership.

I offer an integration of medication management and various psychotherapy. I am certified in supportive psychotherapy, prolonged exposure therapy for PTSD, motivational interviewing, CBT for chronic pain, Integrated Behavioral Couples Therapy, and Whole Health. With the right combination of treatments, patients can achieve their potential and relieve suffering.

In an initial visit, patients can expect a thorough, collaborative discussion with shared decision making to understand their symptoms, life goals, and clinical needs. I will evaluate the patient's condition to determine appropriate medication interventions and discuss personalized treatment recommendations including integrated psychotherapy. Typical follow-up visits are approximately 20-30 minutes in length. Depending on the severity and complexity of a patient's situation, referrals to additional counseling or therapy may also be suggested.

I am a Board-Certified psychiatrist and psychoanalyst with over 30 years' experience in medication management and therapy. I trained and worked in New York City for 20 years and am now living in Hawaii. My training includes Columbia University Vagelos College of Physicians and Surgeons, Columbia University Psychiatry Residency Program (Chief Resident), Columbia University Public Psychiatry Fellowship, and Columbia University Center for Psychoanalytic Training and Research.

  • Virtual faculty practice, inclusive of education, research and engagement opportunities
  • And more
How does compensation work?

Is there a cost to my health system?

Clinicians receive a base salary plus monthly productivity incentives. This structure provides stable income with the ability to increase earnings based on caseload and scheduling preferences. For early-career psychiatrists, it offers a predictable start; for experienced clinicians, it provides scalability with transparency around expectations.

Can I work anywhere?

Is there a cost to my health system?

Do you offer full-time and part-time roles?

Is there a cost to my health system?
