I provide psychiatric services to adults ages 18 and older, focusing on medication management and psychoeducation, often along with supportive therapy. With a patient-centered approach, I believe in fostering a safe and non-judgmental environment where individuals can openly discuss their mental health concerns. I am well-versed in a wide spectrum of psychiatric conditions and offer evidence-based treatments tailored to each patient's unique needs. I have a particular interest in treating mood disorders, anxiety disorders, OCD, and ADHD. I am passionate about reducing the stigma surrounding mental health and strive to provide the highest quality care to help individuals lead happier and more fulfilling lives.

I completed my Psychiatry residency at Rutgers Robert Wood Johnson Medical School in Piscataway, NJ. I received my bachelor's degree in Psychology from Fordham University and then attended medical school at the Lewis Katz School of Medicine at Temple University.

Brian Schultz, MD

Staff Psychiatrist

Dr. Brian Schultz earned his PhD in Molecular and Cellular Biology and his MD from the University of Washington. He completed his psychiatry residency at Northwestern University in Chicago and is board-certified in adult psychiatry. Dr. Schultz has served on the teaching and clinical faculties of the University of Hawai‘i and the University of Washington. His experience includes inpatient psychiatry, outpatient community clinics, cancer treatment centers, and psychiatric consultation in hospital medical settings. He has treated patients from college age through late life and, as the spouse of an active-duty military service member, understands many aspects of military family life.

Dr. Schultz engages with patients in a casual, conversational style and believes that medication, when appropriate, is only one part of a comprehensive treatment plan. He strives to create a supportive, non-judgmental environment where patients feel comfortable sharing their experiences, concerns, and goals. He values honest, open communication and welcomes discussions of the gray areas of diagnosis and treatment. Dr. Schultz encourages active participation in the healing process and tailors care to each individual’s unique story.

Dr. Schultz begins by listening to each patient’s concerns, experiences, goals, and history, followed by a discussion of clinical impressions and treatment options. He uses an evidence-based approach to develop personalized treatment plans, recommending medication when indicated and incorporating other strategies such as cognitive and behavioral techniques. Dr. Schultz collaborates with other healthcare providers when appropriate and recommends additional behavioral health resources when beneficial. Ongoing follow-up focuses on assessing progress and adjusting treatment to support patients’ goals.

Nathan Schulz, MD

Staff Psychiatrist

I value open, honest, and ongoing dialogue about how mental health treatment can improve wellbeing and lead to healing. Over time, I tailor treatment recommendations based on individual needs, which may include medication, therapy, lifestyle changes, or other approaches. I avoid a one-size-fits-all model and work to create an environment where patients feel respected, understood, and supported as unique individuals. In the first meeting, I work to understand each patient's goals, experiences, and preferences. Some arrive knowing what they want, while others are new to exploring mental health challenges. My goal is to meet patients where they are and adjust treatment focus as needs evolve. I emphasize collaborative decision-making, clearly discussing treatment options, pros and cons, and next steps, while ensuring patients feel informed and empowered throughout care.

I attended the Ohio State University College of Medicine, where I received my degree, before completing a residency in psychiatry at the University of Illinois, Chicago. After several years working with veterans at the Jesse Brown VA in Chicago, I returned to my home state of Iowa in 2022. Since then, I have continued to work with a wide variety of clients and conditions in both inpatient and outpatient settings, including at the University of Iowa and Clive Behavioral Health hospital.

David Schwartz, MD

Staff Psychiatrist

I focus on using my extensive medication knowledge to help treat an array of psychiatric disorders. Beyond medication management, I also work on a holistic approach to psychiatry, including working with the patient to get the proper therapy services when clinically indicated. During the initial interview, a thorough assessment is done, including any medical and psychiatric history, medications you may be currently taking, and supplements before any medication is dispensed. My interests include depression, anxiety, ADHD, obsessive compulsive and related disorders, transgender patients on hormone therapies and post-surgical transition. I also have an extensive interest in medical illnesses with a psychiatric component, such as cancer patients, transplant, HIV, and many others alike.

I am a board-certified Staff Psychiatrist at Talkiatry. I completed my residency at Bergen New Bridge Medical Center in Paramus, New Jersey, where I excelled in leadership and knowledge, becoming Chief Resident in just my third year at the busiest outpatient behavioral health clinic in New Jersey. Upon completion of my third year, I went on to become Chief Academic Resident of my program, which allowed me to impart my knowledge of psychiatry to younger psychiatrists in training. After completing my residency in General Psychiatry, I completed a fellowship in the prestigious Mount Sinai Health System in New York City, based primarily out of the Beth Israel location. During this fellowship I studied Consultation-liaison psychiatry, also known as psychosomatic medicine, an exciting subspecialty of psychiatry that focuses on the care of patients with comorbid psychiatric and general medical conditions. Consultation-liaison psychiatry deals with the understanding and advancement of medical science, education, and the provision of healthcare for medically ill and complex patients.

Selda Carter, DO

Staff Psychiatrist

I chose the field of psychiatry with the goal of empowering my patients and equipping them with the skills and tools to improve their mental health. I strive to create a safe, nonjudgmental space where patients feel heard and supported, and to destigmatize mental illness and normalize seeking help when struggling. My passion for psychiatry stems from a deep interest in connecting with people and understanding their unique stories.

I believe the patient-physician relationship should be a partnership, built on trust, empathy, and shared goals. I encourage honesty, collaboration, and open communication, and believe there are no wrong questions, and your voice matters. I encourage questions and feedback and tailor care to each person's goals, comfort level, and pace. I want to get to know the whole person, not just their symptoms. Together, I work with patients to create a personalized and thoughtful treatment plan that may include therapy, lifestyle support, and/or medication, always with room to adjust based on your evolving needs and preferences.

I believe trust is the foundation of meaningful treatment. I aim to create a space where patients feel seen and supported yet thoughtfully challenged, using genuine curiosity, warm humor, humility, and a direct style. I collaborate with each patient to develop holistic, realistic treatment plans that may or may not include medication. My therapeutic work blends psychodynamic and humanistic approaches, with availability for a limited number of weekly psychotherapy patients. In the first appointment, I explore each patient's life story, asking detailed questions to understand them as a whole person. I then focus on their immediate concerns and work collaboratively over time to refine a treatment plan that balances symptom relief with deeper, long-term healing. I think integratively and holistically, considering medical and mental health history, lifestyle, self-narrative, psychological structure, cultural and spiritual background, and broader societal influences.

Seth Sherman, MD

Staff Psychiatrist

At the heart of my clinical approach is a strong emphasis on establishing trust, rapport, and therapeutic alliance to help persons to feel and live well and to progress toward their goals, both in treatment and in life. I begin with consultation, a compassionate, collaborative process of discovery that culminates in a working diagnostic impression and, in turn, treatment recommendations. For those with whom I engage in a treatment relationship, I provide psychoeducation, medication management, and individual psychotherapy, this last modality predominantly drawing from both psychodynamic and behavioral psychological models to tailor treatment to each patient. On some occasions, I may believe that additional therapy is needed and ask that you bring a therapist into your care team to provide the best outcome.

I am a board-certified adult psychiatrist who has been in practice for over ten years. I received my medical degree from Stanford University and went on to complete my residency in psychiatry at Harvard University. After completing training, I cultivated a private practice of psychiatry in San Francisco, California, treating persons with a variety of psychiatric illnesses, while also providing consultation and clinical supervision to psychotherapists in day-treatment and office-based practices. I then served as a psychiatry professor at the Florida Atlantic University Charles E. Schmidt College of Medicine in Florida, where, as Associate Director of the Psychiatry Residency Training Program, I provided psychiatric inpatient treatment while bringing leadership, scholarship, teaching, supervision, and mentoring to psychiatry residents and medical students. My specialty is providing behaviorally oriented trauma-informed care, and I am experienced in managing a multitude of mental health conditions. Karelis Sierra-Cintron, MD

Staff Psychiatrist

I am board-certified in adult psychiatry by the American Board of Psychiatry and Neurology and focus my practice on medication management. I also engage in supportive therapy in a comprehensive way, focusing on the patient as a whole and not just the symptoms. I have a broad understanding of vitamin support as an adjunctive treatment to traditional medication, and I have broadened my understanding of second-line treatment, having been certified in Electroconvulsive Therapy (ECT), conducting clinics in Clozapine and Long-Acting Antipsychotics (LOA), and conducting assessments for patients interested in weight loss surgery. My approach to treatment is comprehensive, and I may consider the addition of therapy, either by referral or by collaborating with an existing therapist, for a more rounded approach and improved outcome. My current areas of focus are adults with Mood Disorders, Anxiety, Co-Occurring Substance disorders, as well as patients with varied concomitant medical problems.

I received my medical degree from the Universidad Central del Caribe, located in Bayamon, Puerto Rico. Upon graduation, I relocated to Philadelphia, where I completed my residency program at Einstein Medical Center. After completing my training, I worked for 8 years in a community mental health clinic, treating an underserved population in all modalities of psychiatric care. Harmeeta Singh, MD

Staff Psychiatrist

I focus on an individualized treatment plan, not only using pharmacotherapy when needed, but also implementing insight-oriented, supportive, motivational, and brief psychotherapy. I am a firm believer in combining psychotherapy with medication management when warranted to manage and treat a wide variety of mental health conditions including, but not limited to, Depressive Disorders, Bipolar Disorders, Psychotic Disorders, Anxiety Disorders, Trauma related disorders, Substance use disorders, Personality disorders, Sleep Disorders, and Eating Disorders. My dynamic approach is client focused, involving the client throughout their treatment and care plan, taking careful consideration to the individual's needs and goals.

I was born and raised in St. Louis, MO and am board certified with ABPN (American Board of Psychiatry & Neurology). Upon completion of my residency through St. Louis University, St. Louis, I joined my parents' Internal Medicine practice. I learned firsthand that implementing mental health awareness, screening, and education into a primary medically focused practice led to more favorable outcomes. Many mental health conditions can manifest as and/or exacerbate a variety of medical/physical conditions and thereby go undiagnosed and untreated. I have worked closely and coordinated care within the medical field and with other specialists in treating and diagnosing comorbid mental health conditions.

While in private practice for over 10 years, I was also an attending and consultant for inpatient mental health care and provided mental health services for residents in various residential care facilities, group homes, and skilled nursing facilities. Over the past 5 years I have consolidated my practice and am presently employed with Compass Health Network in Missouri, helping bridge the gap in mental health care by coordinating and providing care to the underserved community.

I use a practical and realistic approach with patients to assist them in identifying small, attainable goals and then motivating them to achieve those goals. I prefer to work with adults and relish the opportunity to work with patients who are highly motivated for change.

I completed medical school at the University of Arkansas for Medical Sciences in Little Rock, Arkansas in 1995. I then completed a residency in general psychiatry at Emory University in Atlanta, Georgia, followed by a fellowship in forensic psychiatry also at Emory University in Atlanta, Georgia from 1995–2000. I spent ten years practicing forensic psychiatry at a state hospital in Atlanta. I then decided that I wanted to travel and spent eighteen months as a locum tenens psychiatrist primarily practicing in New England. During this time, I met my future husband and we decided together that we eventually wanted to live in the New England area. In 2013, I officially became a telepsychiatrist, which eventually provided me with the opportunity to move to Rhode Island and practice out of the home my husband and I bought there. I initially saw patients via telepsychiatry at a community mental health center in rural Georgia and later took a position as a remote consultant telepsychiatrist at Acadia Hospital in Bangor, Maine.

I use a variety of therapeutic approaches (supportive, CBT, DBT, psychodynamic) in combination with medication management to help patients identify and develop treatment plans towards their own unique goals. Having worked with patients across their lifespans allows me to take a well-rounded approach to identifying issues with my patients, especially in family settings.

I am a double board-certified psychiatrist in Psychiatry and Child and Adolescent Psychiatry. I completed medical school in my home state at Ohio University Heritage College of Osteopathic Medicine, where the focus on mind-body connection now fits seamlessly into holistically minded patient care. DEBRA SOLOMON, MD

My presence at Talkiatry brings my career full circle and back to my greatest joy as a psychiatrist: working directly with patients one-on-one to help them integrate the complexity of their feelings, beliefs, and personal reality in the context of an increasingly complicated world around them. I take a compassionate, individualized, and pragmatic approach to treatment, beginning in the first visit with a full history and discussion with the patient of their symptoms, challenges, and goals. Follow-up visits are tailored to the patient's evolving needs and combine shared insights and tools such as medications and behavioral practices to support them as I guide them on their unique journey toward their best life, on their terms.

My qualifications include a B.A. in biology from Harvard University and an M.D. from Tufts School of Medicine. I completed psychiatry residencies at Mt. Sinai and Brown and am board certified in both Psychiatry and Addiction Medicine. My four decades of practice have exposed me to an eclectic range of cultures, ethnicities, and pathologies in a wide variety of settings, from solo practice to regional medical centers to academic institutions. This breadth and depth of clinical and life experience have helped me transcend cultural biases and institutional conventions and to more clearly see each patient as a uniquely complex and lovable individual.

Thomas Spina, MD

Staff Psychiatrist

I favor a holistic and integrative approach to psychiatry, combining medication management with supportive therapy and elements of cognitive behavioral therapy, while remaining open to other treatment modalities depending on the needs of my patients. When more focused therapy is indicated, I may recommend that my patient work with a provider specializing in a particular modality of therapy as part of their treatment. I treat adult patients with mood disorders including major depressive disorder and bipolar disorder, anxiety spectrum disorders, OCD, PTSD, and specific phobias.

I began my practice at Talkiatry after completing my psychiatry residency at the Eastern Long Island branch of Stony Brook University, where I treated a diverse patient population. I received my undergraduate degree from St. John's University in Queens, where I studied Philosophy. Initially interested in a career in law, I decided to change gears and pursue a career in medicine after watching a relative struggle with her diagnosis of Diabetes Mellitus. I then completed medical school at Tel Aviv University in Israel, where I fell in love with psychiatry during my clerkship at Ichilov Medical Center. During that rotation, I came to appreciate how each patient has his or her own unique story, regardless of their diagnosis.

In my spare time, I enjoy a variety of hobbies, including Brazilian Jiu-Jitsu, running, baseball, and exploring new eateries and shops.

Additionally, I aim to enhance wellness through discussion of lifestyle modifications that emphasize quality sleep, a healthy diet, and regular exercise.

Initial appointments consist of a psychiatric evaluation and discussion of evidence-based treatment options. If telehealth is deemed to be an appropriate modality and pharmacotherapy is recommended, discussion will consist of selecting a suitable medication to address target symptoms while minimizing adverse effects. My approach integrates psychiatric and psychotherapeutic practices with complementary, alternative, and natural therapies, as well as harm reduction. I then acted as a sole proprietor of an outpatient psychiatric medication management and psychotherapy practice in Georgetown, Texas, and later started contracting for North Central Health Care Mental Health Clinic, applying psychotropic medication management expertise to community-based treatment facilities in Wisconsin.

Shilpika Varma, MD

Staff Psychiatrist

I believe in a holistic, integrative approach to mental health. My philosophy centers on identifying the root causes of ailments rather than merely addressing symptoms. I view mental health care as a partnership, where the mind, body, and spirit work together to achieve wellness, and I am committed to fostering a safe, confidential therapeutic alliance. I embrace integrative psychiatry, which combines the best of conventional Western medicine with complementary approaches. This involves a comprehensive biopsychosocial assessment tailored to each individual, incorporating psychopharmacology alongside lifestyle adjustments in diet, movement, and breathwork. I collaborate with therapists and other professionals to create a well-rounded treatment plan that honors each patient's unique journey.

I grew up in an immigrant family surrounded by aspiring doctors, and my passion for understanding the human mind and behavior blossomed early. This curiosity led me to pursue extensive medical training abroad, beginning at Kasturba Medical College in Southern India, followed by clerkships at the University of the West Indies in Trinidad and Tobago. I completed my adult psychiatry residency at the University of Florida in Gainesville and specialized further with a child/adolescent fellowship at Brookdale University Hospital in Brooklyn, NY.
